Forms are based on many things including from what I have seen when you pick your minor. What rank you perfect the spell/skill race and quite a few other things I am sure. To test this a friend and myself both made the exact same chracters except for sphere and ethos. His got a different form than mine. Just putting in my bit of knowledge.
