Don't worry about the line picking up without a ring - probably just the way the PBX's at the ISP are configured (the phone ringing tone is a noise generated by the telco switch for convenience).
I would suspect dirty in-house wiring to be your problem, expecially if different ISP's yield the same sort of problem. In my opinion, a fried modem wouldn't work at all - as compared to performing in a degraded state.
I understand that you move a lot - is it possible to pack up your pc and go to a friend/acquaintance house and try dialing from there? If you are still experiencing the same sort of symptoms, I'm at a loss. If everything runs smoothly elsewhere, it's time to get on the horn and try to convince your telco that your local loop line quality needs to be improved. Good luck!
