Questions:
You have a decent mud client?
Set up good triggers?
You dont have a laggy connection?

Do you know what the skills of your enemies are?
Do you know your way around the areas? ie to implement plan B
plan B is run like a bastard

Its ok to die, just learn from it, and think what you could do better.
EQ helps alot but all of the above is much more important.
