Each time something significant happens with my character I log it with the elven date and write about the event and why it was significant to me. Pk's i mention but i rely more on specific parts of my RP that show a developed character role that differentiates me from other faces in the crowd. So my answer is that I write mine just as one would a journal in real life. Helps me keep a better tab on people too.
