Limited equipment works something like this...
First, there's Unique limited equipment. There can be only one of each item ever (exceptions occur, but usually because of a glitch). Things like this are the wand of Orcus and certain hell items.

Secondly, there are rare items with a fixed number... usually 2 or 3. In theory, these can fluctuate, but probably never will.

Then, there's the standard limited item. The number of these limited items is calculated by taking all the player levels (above 2nd or 5th level, I forget which), so something like an ice-cold broach may be limited to 40 for every 100,000 player levels above 2nd.
Imms control the ratio when the area is made public.

This is a bit of a simplification, but should give you a general idea.
