Alot of it is fairly clearly defined in the help files however to answer some of your questions...

A transmuter who chooses shapeshifting as his or her magicfocus will receive three forms (On major for in their major focus and a minor form in the major focus and one minor form in their minor focus). There are alot of elaborate theories on what form you'll get based on alot of things but you dont really get to say "I want falcon" and that is what you'll get.

A muter can however change into any one of the forms he learns. Hope that helps.
