Sure, it's easy - I know my company hands out root access on DMZ/Extranet servers.
Of course , you may be talking about simply setting up telnetd on some completely external device (like at your home). In this case, I wouldn't be surprised if there was a TCP port 23 access-list in place on the external router and/or firewall. but re-reading your step two I noticed that you suggest assigning telnetd to listen to a nonstandard port, i.e. 80 or 110. Does this actually work??
