I wouldn't write a note that says "Hey! I need me a quest!" I'd begin by introducing the character, and defining why you think you need to interact with that IMM. Usually, this means you want to be a follower, but not always. Have a well-defined character, including description and role, and do not break character. Pray for the IMM's attention. It's like getting empowered, which is a mini-quest in itself. I've found that many IMMs are willing to send you to do something, but you should point them in a direction. Maybe you lost a faimly heirloom in the past (mention it in your role), and wish guidance from your patron deity in finding it. Maybe you have an arch-enemy, and want a blessing to seek this enemy out. Etc. Be creative, and they will often be happy to play along.
Many IMMs will also start quests without much prompting as well, often open to many possible characters. Keep an eye and ear out for strange happenings, and roleplay your interaction with these unusual occurrences.

Yes, but you damn well better have a good IC reason. Alignment changes have been done from time to time (voluntary and otherwise), because a character did a good job of roleplaying someone who undergoes a change in philosophy because of some event or such. Telling an IMM that you seek a special treasure/skill/whatever is usually just a fine way to hang a "Greedy Bastard" flag on you.

Yes, it would depend, but I would imagine that most IMMs get pissy when they go through the trouble of arranging a quest for you, and you decide to run around looking for gear or levels instead.

Speical titles, changes of alignment, change of class, lichdom, quest forms for trannies (gryphon, wolverine, yeti, displacer beast, etc.), quest skills (fireblood, executioner's grace, others), unique items, special tattoos, etc. It's limited only by the IMM's imagination, coding expertise, and game balance. However, note that "big" rewards are reserved for players who do something truly exceptional, and not just roleplay once in a while. Most of them have gone to people who do things like lead/found cabals, play a difficult role despite drawbacks, and generally add to the game by going above and beyond what is expected.
If you're just getting started, begin with the more commonly seen quests: Get a tattoo, earn a title, roleplay something a little different. The IMM staff is good about encouraging newer players who clearly try hard, and bring quality roleplaying to Thera. They're also quite good at destroying characters who are conspicuously bad at roleplaying. Put in the effort, and the rewards will come. Roleplay even when you think no one is looking, mortal or otherwise. Treat mobs as you would other characters. Explore areas thoroughly, and look for things that would be meaningful to a character of your background. Be consistent.
