In my experience (and I did actually experiment extensively) My human mace spec with 23 str and 20 dex never once lagged an opponent with Cranial.

Looking over logs of Dwarven mace specs...their opponents always get at least one chance to do something before the dwarf can cranial again.

I have seen logs of cloud giant mace specs using cranial where the mage doesnt get any actions however.

I am thinking that cranial lagging is str based, and the 23 humans get (let alone the 21 dwarves get) just isnt enough raw strength to lag an opponent totally.
