Whether you hit or not with your ambush seems to be tied pretty much directly into your ambush skill. The amount of damage, however, doesn't seem to be tied THAT much into it. I'm not sure if this helps, but I'd say that the times when you "miss" are just bad luck (you failed your % check on ambush). As for when you get really weak damage...uh, no clue.
