That's not really true. Here are a few examples... although I agree, most my deaths were from situations where i knew I would probably die.

Slept by bard. Held. Insects. Eagle and bard attack.

Slept by necromancer. Forgotten. Insected. Held. Killed by bunch of lunkheads. Odd combination, but did happen.

Knocked out with fists. Sanc falls. One round death to deathblowing warrior.

Blackjacked. Fly falls. Cheap shotted and tripped to death.

I'm not saying I was the best healer, but I was decent, always aware of my protections, always ready... but some things you just can't prepare for.
