I could be wrong but I think hide is pefected, dodge etc arent... only because as my cat I never failed hide as far as I remember.

You revert to human form because you cant speak in your form. you cant cast some of the spells, people get to see what forms you have which is not often a good idea, and you loose mana every tick.

Your are also pretty limited to what you can do in your form as far as social interaction as the socials list is small. and you cant emote (pretty sure thats correct.. else emote says whatever could be abused).
