Quest forms are usually a reward for extremely good roleplaying. They are very rare, and as stated in the help file, the best way to assure yourself not to get one, is to ask for one.
I have not personally seen a quest form in the game, but ive read the logs on them.
Your roleplaying doesnt have to be good...it has to be awesome for you to be considered.
