1) Your rank - lotsa mobs are just not agro for you if you are hero.
Teleport/spellcraft percentage - I think it is all wrong here.
Your luck - go figure.
World restriction : You cant travel across continents or time with teleport.
Area restriction : You could not teleport to explore_areas, shrines and areas you can't enter in normal way (for example: Mud School, Skies above Thera).
Room restriction : Room you land into can't be norecall, noexit or underwater. You cant land into private, god_only rooms.
No_magic, locked but recall room is very possible to land into (room past Sko in Balator). So keep 'pass door' up :-)
I do not know rooms what are no_teleport but recall.
Like : udgaard, eryn galen, AoN, dol guldar, aryth ocean, eryn galen, emerald, ugdaard, eastern, mirkwood, open planes, desert of araile, eryn galen, etc.
In game merchanic terms it basically means what random function what chooses location for you is a bit too pseudo_random.
