You used to see it much more often back when CF was extremely unstable. For example, an item could be duplicated in a crash. (See above) Item limits are based on characters, so the constant flow of deletions and character creations will cause them to go up and down slightly like the tide. Another possibility is pfile corruptions; sometimes if your file goes corrupt it will be the case that some or all of your equipment isn't being counted as "out" at that time. Meanwhile, someone goes and snags that ragonin the cleaver you had, and then when your pfile is restored it's one over limit.
