I saw this sort of thing all the time (with disrupts especially) with my last transmuter. I have a strong hunch that some of the transmuter spell lags on the caster are measured in pulses (like trip lag on the trippee- slightly over or under an even number of rounds). If it makes any sense at all mental jolt may be like 1.1 rounds. So when it falls on the round that melee will go through and the next round of melee will "beat" your next command. Most of the time they come out with a consistent number of melee rounds in between but when they fall on or real near the end/beginning of a round you get weird results.
On the other hand I may also be on crack. Take that observation worth a grain of salt and try it out yourself sometime. I'll try and dig up some logs maybe if I get bored.
