Recently had some shit connection. Like you describe, big spikes of lag crap. Turns out the firmware of my router/modem was super out of date. Manufacturer says call ISP. ISP says we don't update that shit, we just give people new ones if they rent. I don't rent, because renting is for turds. So I guess, check your firmware? You sound like someone that would have already done this though. *shrug*
