What special forms have been given?

I know of the Wolverine, the Displacer Beast, the Cheetah, the Bullette, the Griffon and the Yeti.

Are there others? Is there a quest form for each focus? (offense, defense, water, utility and air)

How does one go about getting one? Is there a built in quest?
