To gain insight into it you need some skill in hth, elbow, knee, kick.
From the helpfile, brawling is an automatic skill that helps unarmed attacks do more damage, and may also help in defending against these attacks.
Once you have it, it'll perfect fairly quickly, which sort of lends to Nurok being correct that it helps against defenses, since most mobs have unarmed attacks.
