Looks to me like an echo from equipment being removed on the death.

See similar things from logs with say troll amulet.

Someone is DEAD!!
You land the killing blow against someone.
Someone's head is shattered, and its brains splash all over you.
Finduthil looks strangely pained as his amulet stops glowing.

No, it's a necklace that poisons you when you remove it.

That's why I asked, there've been buggy items in the past giving you a fatal affect even though you just became a ghost. (n/t)
