Question regarding Soul (and no, this has nothing to do with the log)

Per the legacy page, Soul sometimes gives a surge when near death. If this truly exists, is this combat only, or can it occur due to some sort of maladictory effect like bleeding or poison?

The effect belongs to "Spirit of the Blade" (I think), and has to be combat damage -nt-
