I am not sure of all the things that factor into the "blood spurt" effect, but I am pretty sure these elements are a part of them:
1) It has to be from a bladed weapon - the heavier the better. Slice, cleave, etc. may be important. Axes and polearms are best.
2) Strength is important.
3) It is most likely to happen from attacks that boost damage (I believe pincer has a small multiplier effect on its damage, for example).
That could all be crap...I don't know. Just what I (think) I've observed.
The result of the "blood spurt" is a puddle of blood on the ground. This is most often never useful. However, you can sense magic in the blood and, if I recall correctly, alignment.
