I've seen it fail when someone tries to vanish from combat quite a few times. I'm not sure I'd say it's a reliable way to get out of combat, but once you flee, yeah, vanish makes it very hard to re-engage. I used it for very fast travel, like you suggest. Getting to areas and getting close to an enemy in an area before he knows you're there.

Not just surviving and killing, but the time factor. So, you have to be able to take punishment and heal, but you also have to deal a lot of damage in a short amount of time.
I'd think the white and red would be the best candidates because of the vuln to cold/fire the mobs have. Maybe a Maran Paladin would have the best shot in the red lair.
