I'll only post the ones that I remembered the most:

Lygram - Gnome Shifter - One of my first chars, went align change to good, and Fort. Lost interest after trying to get spells back.
Moroni - Fort Shifter
Lehi - Fort Assassin
Drogal - Fort Conjurer

There have been a bunch more failures here, not that I'm saying any of these were partularly successful. Moroni actually got to his final forms, Lehi could have hero'd but I lost interest due to lack of real PK skill on my part, and got smacked by the Grobbak for picking names from Mormon Scriptures.
