Depends on the level and who you're picking on. As was pointed out, you're good, so fewer targets.

The tattoo is great every once in a while, but when I had it it maybe gained me 2 kills (out of ~75) over the course of the character's life.

Its usefulness is tempered by the fact that, since people know you have it, they tend to be more cautious and start fleeing earlier.
