While I don't want to totally dismiss a real medical condition, I guess I'm agreeing with Stevers *shiver*.

I suppose you mean you can't focus yourself on one thing? Either way, being able to multitask in some form, is great. Just keep attentive so that you don't end up doing a bad job at both. Focus primarily on one first, than the other.
- I can't sleep because I always think (I always take an hour or two before I finally sleep)
Go do some heavy sports activity (like martial arts and/or running) and get back like an hour before bedtime (to let the adrenaline cool down). Drink something warm while taking it easy*, like soup or tea, eat, but only a little and something healthy. Nothing with caffeine, that's also most sport drinks and Red Bull and the like.
* Watch some tv, read a book. Don't go sit behind your computer.
- I am not able to be structured at all
Learn yourself some self-discipline. Make an effort to arrange things in a structured manner.
- Can't read because I can't focus
- After 30 seconds, my vision gets blur and I am unable to understand fully what people speak from now on.
Are you already on medication? If yes, see about getting the dose reduced.
Don't sit behind the computer for extented time periods. Check with a specialist if you need glasses.
