Should the boot quest be offered to good/neutral only?
I don't think so. Saving one creature for your own benifit is just as nasty as killing one for your own. Sure... You saved a creature's life. It's eternally greatful. Who cares? You don't. You got what you came for. The creatures of the quest shouldn't need to care whether the savior is good or evil. If the good witch or the bad witch could save me from something, it wouldn't matter to me which one did. The reward...? It should also be given, reguardless of alignment.

Should Imperials be Anathed for taking the quest?
I don't think so. (I'm no officianado of the Empire, mind you.) What is the Empire? A group of evils with similar ideals. To quote them, they go after their goal "By any means nessicary." So? They save a good creature & get a reward. Did this change their ideals about lawfulness or order? Not particularly. They saw a chance to benifit and they took it. Doing so in no way changed the Anath's morals nor did it detract from the beliefs of the Empire. If anything, having the reward may have made the Empire stronger. How taking the quest can be a violation on "My blood, My Soul, My heart for the Empire" is beyond my comprehension.

Should taking the quest have any affect on an evil character through an Imm's eyes?
Only if they consider this blatant eq hogging of sorts and change the character to neutral.

Should the boot quest be removed?
Probably... Too many people know, it seems. I could be wrong though. Maybe just moved/replaced.
