vs. Zwerg. After his entwine, your next move should be a lag or a blind move, to take advantage of the fact that he has no melee.

Also, when you can input commands, go ahead and do it. A few jabs or crosses would have helped.

Gather elemental type damage swords. Flurry is the bane of minotaurs because they can't wimpy out of it.

vs. Zerrla. You need divine saves and about +10 str or more. You need light maces (long elvish mace etc). If shaman is prepped just get out. A good, geared shaman who immediately spams -str on you will screw you over, you don't have a choice there. Your only chance is to maintain high mobility, get out if str drops, and do try for bash/trip when you can. Check shaman's headgear before using cranial.

Also, if there are Centurions, don't fight unless absolutely sure you can kill the Cents before they arrive.
