With respect to Car Insurance vs. Healthcare Insurance, Car Insurance is necessary because it affects other citizens. Thus, it can be argued that it is used for the 'benefit and protection' of your fellow citizens.

Healthcare is really only about yourself.

I think that's the clear distinction made - it's about whether the government has the right to enforce a requirement to obtain an arguably unnecessary aspect of living in US that does not affect fellow citizens directly.
