By "I don't want to go for anything but sneeze", I mean that they won't be competent enough to treat something serious. Simply because rural doctors (even if they have necessary education) don't have enough practice with serious illness.
Like in Moscow, we have a medical research institute for any kind of illness. So if you've got shapamatronic syndrome, chances are we have a "Federal Research Institute for shapamatronic syndrome", and people there cure tens of thousands of it each year. So I'll head right there if I get it. And guess what, I can do it for free :)
In rural areas, chances are the doctor has never seen shapamatronic syndrome in all of his life, and I'm not inclined to become his first patient, be it free or paid or whatever.
