For a programming assignment I ran out of time and I ended up cleverly commenting out some garbage collection code that wouldn't compile. The professor never noticed.
Another time for a business/accounting type class I programmed some equations and definitions into my calculator for an exam.
Those are the only ones I can think of. It's been 15 years. Now to answer your questions.

For me it was probably more of a risk vs reward thing than what it was on.

I wouldn't cheat anymore, but at the time it was probably about the same.

Don't know. Haven't thought about it. Should probably find out how prevalent it is first.

I'm not okay with cheating, but I think the prospect of cheating is factored into the value of a degree. I have heard fraud is showing up more and more on resumes. It does matter how much cheating is happening. I have heard of teachers in high schools helping their students cheat to pass the standardized tests. If cheating is reaching systemic levels, something should be done. I'm really not expert in this area.
